Thank you for taking the time to register your child(ren), newborn to 5th Grade, for the 2024-2025 school year starting on August 11, 2024. Registration is required to attend Saturday Kids Fellowship and Sunday Kids Church activities. Please complete this form in English.

Each registration must provide information of AT LEAST ONE parent / guardian. Providing information of the second parent/guardian will be best in case the first parent/guardian cannot be reached. Only email messages about Kids Church events and important announcements will be sent to the provided email addresses. The phone number is necessary to contact the parent / guardian in case of an emergency. For form fields that are not applicable, please enter "NA".

Release of Liability: I give permission for my child(ren) to take part in SJCAC Children's Ministry activities. I understand that there is a risk of injury arising from participation which may be caused by the child(ren)'s conduct, the conduct of others participating in the activities, the alleged conduct of the activity operators, or the conditions in which the activities take place. I am expressly assuming the risk of any physical or mental injury and COVID-19 exposure that may arise in association with the child(ren)'s participation.
